Here at Platinum Salon by Merlande, having you as a client is very important to us. We strive to bring you the "Platinum Experience" every time you visit. In order to do so, we have implemented salon policies. Please be sure to read all of our policies before your next appointment as they may change.

Coronavirus/COVID-19 Salon Consent and Waiver:
RELEASE OF LIABILITY AND AGREEMENT NOT TO SUE, INDEMNIFICATION, HOLD HARMLESS, LIMITATION OF WARRANTY

 

By booking an appointment at Platinum Salon by Merlande, LLC you acknowledge and agree to the following:

• I AGREE that by booking my appointment for haircuts, extensions, chemical services and/or any service, I accept all responsibility for the risk that I may contract COVID-19.

• I ACKNOWLEDGE and AGREE with the contagious nature of the COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

• I ACKNOWLEDGE and AGREE that I must comply with all set policies, pre and day of appointment procedures as outlined on merlande.com/covid-19 to reduce the spread while attending my appointment.

• I hereby RELEASE and AGREE to hold Merlande Petithomme, Platinum Salon by Merlande, LLC harmless and from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Platinum Salon by Merlande, LLC.

• I ACKNOWLEDGE and AGREE that this release discharges Merlande Petithomme, Platinum Salon by Merlande, LLC from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Platinum Salon by Merlande, LLC. This liability waiver and release extends to the salon together with all owners, partners, and employees.

• I ACKNOWLEDGE and AGREE that I am free to go to other salons who may not require my agreement to accept responsibility for contracting COVID-19 and I chose to have hair services performed including but not limited to haircuts, extensions, and chemical services.

 

 UPDATED PLATINUM SALON BY MERLANDE, LLC SALON POLICIES

By booking an appointment at Platinum Salon by Merlande, LLC you acknowledge and agree to the following:

• I ACKNOWLEDGE and AGREE that Merlande Petithomme, Platinum Salon by Merlande, LLC explained to me what is the plan of treatment, the benefits, the pros, and cons.

• I ACKNOWLEDGE and AGREE that Merlande Petithomme, Platinum Salon by Merlande, LLC and any employees of the salon will not be responsible or liable if the result of the service is not as expected as it should be.

• I ACKNOWLEDGE and AGREE that I am allowing Merlande Petithomme, Platinum Salon by Merlande, LLC and any employees of the salon to apply necessary chemicals as part of the service in my hair treatment (if chemical services are being performed).

• I ACKNOWLEDGE and AGREE that if I am more than 15 minutes late, I will be imposed with a $15 fee and if I arrive more than 30 minutes late, the appointment will be forfeited and the original appointment rescheduled.

• I ACKNOWLEDGE and AGREE that if I will provide a minimum 24-hour notice if I would like to cancel or reschedule the appointment.

• I ACKNOWLEDGE and AGREE that if I am a no-show without any prior notice, I will be charged a no-show fee of up to 100% of the service value.

• I ACKNOWLEDGE and AGREE that my deposit is non-refundable and cannot be transferred to a future appointment.

• I ACKNOWLEDGE and AGREE that if I am not completely satisfied with my service due to an error or mistake, I have 24 hours after my service to contact Merlande Petithomme, Platinum Salon by Merlande, LLC and any employees of the salon via email at info@merlande.com. Changing my mind after the service has been performed is not deemed an error or mistake by the service provider.

• I ACKNOWLEDGE and AGREE that all services performed by Merlande Petithomme, Platinum Salon by Merlande, LLC and any employees are final and are non-refundable.

• I ACKNOWLEDGE and AGREE for Merlande Petithomme, Platinum Salon by Merlande, LLC, its employees and contracted employees to use my image, voice and/or likeness for print and digital media marketing campaigns, testimonials, media, technology now known or hereafter as long as there is no intent to use it in a disparaging manner.

• I ACKNOWLEDGE and AGREE that kids are not allowed in the salon unless they are being serviced.

• I ACKNOWLEDGE and AGREE that Merlande Petithomme, Platinum Salon by Merlande, LLC and any of its employees have the right to refuse service to me if my behavior is inappropriate and negatively the effects of the service.

• I ACKNOWLEDGE and AGREE that no pets are allowed in the salon unless they are service animals.

• I ACKNOWLEDGE and AGREE that I cannot bring a guest unless for health reasons or accompanying a minor.

Payment Options

At this time, we are only accepting contactless payment options such as major credit cards or debit cards, Apple Pay, Google Pay, and Platinum Salon by Merlande gift cards due to COVID-19. NO Cash, CashApp, Zelle, PayPal or any other similar payment methods will be accepted.

Late Arrivals

I ACKNOWLEDGE and AGREE that if I am more than 15 minutes late, I will be imposed with a $15 fee and if I arrive more than 30 minutes late, the appointment will be forfeited and the original appointment rescheduled.

Cancellations​​

I ACKNOWLEDGE and AGREE that if I will provide a minimum 24-hour notice if I would like to cancel or reschedule the appointment. Please note: We do understand that emergencies happen that are unavoidable. In this case, we will do our best to work with you.

No-Shows​​

I ACKNOWLEDGE and AGREE that if I am a no-show without any prior notice, I will be charged a no-show fee of up to 100% of the service value.

Deposits

I ACKNOWLEDGE and AGREE that my deposit is non-refundable and cannot be transferred to a future appointment.

Refunds​​

I ACKNOWLEDGE and AGREE that all services performed by Merlande Petithomme, Platinum Salon by Merlande, LLC and any employees are final and are non-refundable.

 

Errors

I ACKNOWLEDGE and AGREE that if I am not completely satisfied with my service due to an error or mistake, I have 24 hours after my service to contact Merlande Petithomme, Platinum Salon by Merlande, LLC and any employees of the salon via email at info@merlande.com. Changing my mind after the service has been performed is not deemed an error or mistake by the service provider.

Promotional Release

I ACKNOWLEDGE and AGREE for Merlande Petithomme, Platinum Salon by Merlande, LLC, its employees and contracted employees to use my image, voice and/or likeness for print and digital media marketing campaigns, testimonials, media, technology now known or hereafter as long as there is no intent to use it in a disparaging manner.

Health Issues

For your comfort and safety, please notify our service providers if you have allergies, any physical issues or disabilities, or if you are pregnant. If you experience discomfort or any other symptoms during the treatment, please alert your service provider immediately.

Right to Refuse Service

I ACKNOWLEDGE and AGREE that Merlande Petithomme, Platinum Salon by Merlande, LLC and any of its employees have the right to refuse service to me if my behavior is inappropriate and negatively the effects of the service. This includes but is not limited to behaving improperly, being intoxicated, using excessive vulgar language.

Consultations

Any new client, bridal, wigs or coloring services will require a consultation. The consultation fee charged will be deducted from your total service charge if you decide to move forward with our services. All consultation fees are non-refundable.

Additional Policies

  • I ACKNOWLEDGE and AGREE that kids are not allowed in the salon unless they are being serviced.

  • I ACKNOWLEDGE and AGREE that no pets are allowed in the salon unless they are service animals.

  • I ACKNOWLEDGE and AGREE that I cannot bring a guest unless for health reasons or accompanying a minor.
